S-3B??????/AA-712 VS-30USS SaratogaPlane folded up her gear while sitting on the fan tail one day waiting to be launched, screwed up the recovery big time. Had to get Tilly crane out to pick her up and drop her gear. When she folded up her gear the gear pins had been pulled but there are interlocks that are supposed to prevent the gear from being raised while she sitting on deck, guess they didn't work. Some of the antenna's on the belly were driven up through her bottom and the gear doors were destroyed. 14/02/1991 offloaded at Jeddah, Saudi Arabia. Sent back to USA onboard USS Yellowstone AD-41.

HSL-48 After completing a surface surveillance and mine search mission in the north Arabian Gulf, the SH-60B headed back to USS Hawes FFG-53. Pilots noticed a transient No.2 engine chip light, followed by a muffled explosion and engine failure. After going through emergency procedures, Lt. Flanagan flew the helicopter to the nearest ready deck, USS Paul F. Fster DD-964. Lt. Szmed briefed his crew, coordinated with USS Foster, and jettisoned non essential stores. He also dumped fuel and determined the best single-engine approach and landing profiles. AW1 Wiley and AW3 Opp completed checklists for emergency landing-ditching and provided radar vectors to their pilots. Meanwhile, USS Foster quickly gave best winds across her deck as she maneuvered through a known mine area. With Lt. Szmed providing direction, Lt. Flanagan made a single-engine approach and landing.

HMM-774 While leading a section of CH-46Es on a NVG training flight in Saudi Arabia, one of the Sea Knight´s engines suddenly failed during cruise flight at 100 feet and 100 KIAS. Capt. Bullard immediately called for PMS OFF and turned into the wind, setting upa for an emergency landing in the desert. Maj. Gordon confirmed that the No.2 engine was off-line and called out airspeeds and altitudes until 25 feet when Cpl. Steelman took over and cleared the pilot to land. Capt. Bullard made a single-engine no-hover landing. The cause of the engine failure was the loss of the accessory drive shaft. The engine was changed in the dessert the next morning.

HMH-465King Abdul Aziz AB, Saudi ArabiaCrashed during a take off out of a Forward Area Refueling Point (FARP) after taking on fuel for a follow on mission. The aircraft commander had pulled No 1 and No 3 engines back to try and cool them off while taking on fuel. Pressure refueling a CH-53E is a long process, especially from a FARP as the pressure barely keeps up with the fuel the helo is burning sitting there. Pilot taxied out from the FARP and did not bring No 1 and No 2 engines back up to 100% and attempted a take off in high heat environment and a heavy condition being freshly fueled with only No 3 engine at 100%. The helo crashed and burnt to bits. All survived.

335th TFS/4th TFW The aircraft was flying a low-level training mission over the desert about 63 miles north of Thumrait, Oman in preparation for Operation Desert Storm. Due to sand dust floating in the air close to the ground, sand-pitted aircraft windscreen and three external fuel tanks (which was unusual to Strike Eagles at that time), the pilot made a mistake by visually judging aircraft altitude that would have been necessary for performing a split-S maneuver. At the bottom part of the split-S, when the aircraft's nose was already pointing upwards (with the stick being pulled back fully) the aircraft's tail touched the ground (creating a fifty-foot trail of burned sand in the desert) resulting in a crash.

Company B, 7th Btn, 101st Avn (101st Abn Div) The aircraft was the trail aircraft and suffered a maintenance problem/emergency called an Engine Transmission Hot. The oil system had basically failed and the 100+ lb gear box mounted on the front of the right side engine cooked and quickly caught fire. The crew landed the aircraft safely and all 15+ people got out (in record time). The rest of the flight landed near by and for the next hour we watched as the aircraft, a HUMMV, a 105mm howitzer, and a basic load of artillery shells was completely consumed by the fire (and a few small explosions.)

3/160 SOAR Reported launching of a surface-to-air missile. While executing an evasive maneuver, the helicopter struck the ground, shearing off its left-front and right-rear landing gear. When the Chinook arrived at Rafha, maintenance crews worked feverishly to place mattresses and tires under the fuselage in an attempt to keep the aircraft upright during shutdown.. Never flew again in the war.

35th TFW1nm N KKMC The air refueling receptacle on top of the jet failed and they couldn’t take on post strike fuel to make it back to the island. They tried for the emergency divert field to the Iraqi border and the engines flames out short of the field. Crew got out okay and was flying combat missions again after a few days.

It undervent 5 assymetric Gs during flight resulting in all four engines over-torqued, all four propellers leaking fluid and numerous popped rivets in the wings. The plane made it back to King Fahd IAP. A week later, after using all spare propellers, it flew to Rhein-Mein AB for in-depth inspection, and did not return back to Gulf until 19/02/91.

During repositioning pilot transferred controls to co-pilot to send updates half-quick to other aircraft, during final repositioning turn aircraft descended at an excessive rate, Pilot immediately after seeing this in HDU leveled aircraft and pulled max collect to the stop. Aircraft impacted around 12G's, crushing gear and gun, wings separated from fuselage, Hellfires and pylons broke free. Blades never hit ground with max pitch, aircraft was on ground full pitch, both engines TGT LIMITED at 868. Aircraft was shut down, fuselage bulkheads cracked.

42nd ECS/66th ECWnear Saudi Arabia borderCause 1 - possibly locked up in error by a coalition fighter, crew assumed that they were being tailed by a MiG and took the normal precaution of diving to the lowest feasible altitude. Tragically, their aicraft struck the desert, leaving a long streak in the sand with the engines at its conclusion. The crew died in an unsuccessful ejection attempt.
Cause 2 - intercepted by an Iraqi Mirage F-1 flown by Captain Nafie Al-Jubouri, took off from Saad AB better known as H-2 and headed directly to it, fired one single missile type Matra 530, the plane tried to avoid the hit but the missile blew up and caused damage sent the plane to the ground.
Cause 3 - "friendly fire" incident, shot down by a USAF F-15, which mis-identified it as a hostile aircraft (probably an Iraqi Mirage)

15th Sqn. (RAF)50nm NW BaghdadJust prior to weapon delivery the crew received radar warnings but with only seconds to weapons release, continued with the attack. Only one of the two LGBs was released, with the other remaining hung-up. Shortly after, the Buccaneer crew reported a visual sighting of two SAMs fired from a site to the North of the target. Crew attempted to take evasive action and to jettison external stores. Despite the crew's actions a missile exploded beside the aircraft, rupturing a canopy beside the pilot's head and smashing the majority of front cockpit instruments. Pilot could not contact his navigator. Immediately after, there was another explosion from a second missile. Despite the extensive damage, including both wings being peppered with holes and no apparent control over the engines, the pilot still had limited control and flew the aircraft for a further two or so minutes before initiating command ejection when he found he could control the aircraft no longer. The pilot sustained a minor injury to his left leg as a result of ejection and descent. WSO was found to be dead. Pilot was subsequently captured and remained as a POW until cessation of hostilities.

A/C was losing hydraulic fluid and after a successful trap, did not have brakes after disengaging the arresting cable. The tailhook was down and subsequently the nose-wheel steering was centered. Pilot could neither retract the tailhook, steer or stop the A/C due to the fluid loss. As the A/C approached the end of the deck, unable to stop, the pilot and BN ejected. The A/C nosed over the deck, coming to rest tail-up without going completely over. At this point the rest of the returning strike is in the pattern and low on gas. All available deck crew were summoned to push the A/C overboard to clear the deck so the rest of the strike could recover. The A/C was by all means repairable (it flew back to the ship) but time did not permit the use of the crane to move it out of the way.

He was able to eject seconds after the hydraulics gave out and Harrier turned upside down. He was collected by USMC vehicle and returned to duty.

1/227th AHB/1st Cav Div Shot down by SAM, crew used their special harness and took a ride on another Apache. The smoke, combined with the uncertain tactical situation, made a recovery of the downed AH-64A by a CH-47D Chinook too risky. The helicopter was destroyed in place with the launching of two TOW missiles in order to prevent the possible salvage and analysis of secret, onboard weapon systems by any enemy country.

Shortly after entering the fog the helicopter apparently flew into the ground and was destroyed. The accident happened in daylight (08:51 Local time). It is understood that, due to conflicting traffic inbound to Dhahran, the aircraft was instructed to remain below 150 feet until clear of the airport approach path.

Mission was a flight of three (2 AH64s/1 UH60) being vectored to last known location of downed F-16 by AWACS. Flight was vectored directly over a heavily defended Iraqi Republican Guard position. 4/1 AVN A/C took off from Kuwait Two at 1230 hrs with UH-60A 79-23338 on an EPW mission. At 1730 hrs, 78-23015 separated from 79-23338 in order to pickup PAX waiting at Iraq One. A/C crashed on a heading of 340 deg with wreckage downwind of the crashsite indicated inflight break up. A/C was hauling the crew of UH-1H 64-14273 back to port and then jumped back into Kuwait when they were misoriented and shot down by a ZSU-23-2 AA gun from Republican Guard.

02/03/91UH-1H67-171615 injured6th Btn, 101st Avn In the desert when you take off the aircraft is enveloped in a cloud of dust with no visual reference. The technique is to maintain the aircraft attitude while applying torque as necessary up to maximum until clear of the dust cloud. The pilot had applied the torque and was about above the the dirt with little or no airspeed when the engine failed. This altitude does not allow much reduction of the collective because of high rates of decent. The pilot did the best he could with the rotor he had remaining and touched down tail low allowing the tailboom, then the aft fuselage and landing gear to help dissipate the downward g-force. That is what saved them. The ripping up of the rest of the body occurred because the aircraft bounced up and spun around. The main injury happend because the right seat shoulder seat belt (recoil harness) failed and the pilot ate the cyclic and right door post. Broken jaw and teeth.

05/04/91SA-342M3474/94/CXM 1er RHC, French AF Belonging to the 1er RHC but allocated to the RHC Daguet in Saudi Arabia, crashed due to an engine surge on take off. The crew (1er RHC as well) escaped unhurt but the Gazelle was a complete loss, later used as a ground instruction airframe in Bourges. This machine was the third SA 342M from the batch produced for the ALAT, having been originally delivered on the 10 april 1980.

LtCol. Faizz Abdullah Al Reshidi was a Kuwait Air Force pilot, flying Gazelle anti-tank helicopter. He studied in a flight school located in Seattle, Tacoma, USA to become a commercial pilot. Once graduated he came back to Kuwait and applied to Kuwait Airways being rejected as at that time Kuwait Airways was hiring only british qualified pilots. Al Reshidi continued his studies in another flight school in Jordan and earned his private pilot licence from "The Hashemite Kingdom of Jordan Civil Aviation Authority" on June 23rd, 1984. After this licence he came back to Kuwait and decided to join the Air Force. Due to his skills and licences he had been selected to fly and operate the Aerospatiale "Gazelle" helicopter. August 2nd, 1990, Iraqi forces started the invasion of Kuwait and quickly overrun its defences. Iraqi fighters took off from their bases in Iraq crossing Kuwaiti airspace, trying to evade radars and ignoring autharizations from Kuwaiti tower controls of Ahmed Al Jaber and Ali Al Salem. At 11:00am scrambled the first group of Kuwaiti first line of quick alert fighters consisting of F-1CK Mirage being soon outnumbered and overrun. The second wave of fighters took off from Ahmed Al Jaber air base and consisted of A-4KU Skyhawks that intercepted and downed some Iraqi helicopters. At midnight Kuwait Air Force called at arms all the pilots, even those who were out for health reasons or out of the country. Among those pilots was LtCol. Al Reshidi. He was requested to join his squadron at 1:00am. At 2:00am he took off with his unit to intercept and attack enemy helicopters, tanks and troops invading Kuwait soil. His collegues said "He fought like a warrior" and also he was the only Kuwaiti pilot who operated the Gazelle in a way no one could even imagine and he was well known for his sharpshooting skills being able to fire in movement without hovering as most of helicopters do. He took down numbers of Iraqi helicopters and sniped out most of the tanks that was on his path. He came back to his airbase to refuel and rearm, but his squadron was requested to fuel and fly to the near kingdom of Saudi Arabia. The Iraqi waves were non-stopping and Kuwaiti forces started to run out of ammunition with no chances to defende their country. LtCol. Al Reshidi refused to withdraw from the line, but his Gazelle was used to evacuate the Royal family members to Saudi Arabia. So he joined his colleagues in a Super Puma helicopter but soon realized there was no way to make it back home as Iraqi forces were taking partial control of the country. So they headed to Ahmed Al Jaber airbase to pick up pilots left behind and rejoin in Saudi. On the way to the airbase the ATC at Ali Al Salem and Ahmed Al Jaber lost contact with the SA-330H Puma - it was shot down by friendly fire of I-HAWK, one of Kuwaiti anti-aricraft battery, killing all 4 crew members.

Lori S. Stewart, USAICoE Command Historian - On 27 February 1991, Iraqi ground fire brought down a UH-60A Black Hawk helicopter, killing all eight soldiers on board. One of those killed was Sgt. Lee A. Belas, an electronic warfare voice interceptor assigned to D Company, 1st Aviation Battalion (later redesignated D Company, 4th Battalion [Provisional], 1st Aviation Regiment), 1st Infantry Division. Born in Burbank, California, in May 1968, Lee Arthur Belas moved to Port Orchard, Washington, with his family while still a child. As an outgoing and high-achieving teenager, he was on the swim team, in the drama club, and played trumpet in the high school band. He studied the French language in high school also, but after graduation, while living as a Rotary Club exchange student in Belgium, he truly found his passion for learning languages. Before returning to the United States, he had studied German, Latin, and Flemish. With this love of languages, he dreamed of one day working for the State Department as an ambassador. On 14 October 1987, Belas enlisted in the U.S. Army, which promised him training at the Defense Language Institute Foreign Language Center (DLIFLC) in Monterey, California. He graduated from the forty-seven-week Russian basic course in February 1989. He then transferred to Goodfellow Air Force Base, Texas, where he was one of nearly one thousand Army students enrolled annually in the 98G electronic warfare voice interceptor course. Upon completing his advanced individual training and airborne school, Belas was assigned to the 1st Infantry Division at Fort Riley, Kansas. Initially in the division’s 101st MI Battalion, Belas learned about the QUICKFIX intercept and jamming systems mounted on EH-60A helicopters. Intrigued, he volunteered for QUICKFIX duty and was reassigned to D Company, 1st Aviation Battalion, and sent to Fort Huachuca for QUICKFIX flight training. Upon his promotion to sergeant in September 1990, he was serving in two positions. He was assistant team chief and jamming team crew member in the Collection and Jamming Platoon, A Company, 101st MI Battalion. He was also a crew member of the QUICKFIX Flight Platoon, D Company, 1st Aviation Battalion. (As in other active Army divisions, the QUICKFIX company was under the operational control of the division’s MI Battalion.) At this point in his short Army career, the ambitious 22-year-old Belas could not decide which he wanted more: to learn additional languages or to become a Black Hawk pilot. After Iraq’s invasion of neighboring Kuwait in August 1990, the U.S. Army began preparing for what would be known as Operations DESERT SHIELD and DESERT STORM. To prepare for deployment, in November, Sergeant Belas joined all Army linguists at Fort Riley in DLIFLC’s basic Arabic language training via teleconference. The 1st Infantry Division’s deployment came on 8 January 1991. Over the next six weeks, Sergeant Belas clocked forty-six flight hours in the QUICKFIX, monitoring Iraqi communications and movements for 1st Infantry Division and VII Corps ground forces. Around noon on 27 February 1991, just one day before a ceasefire was declared, he boarded a UH-60A in Kuwait that was transporting troops and supplies between assembly points. At about 1730, the helicopter inadvertently flew over Republican Guard forces and was shot down by an enemy ZSU-23-2 antiaircraft gun. All eight personnel on board were killed. In addition to Belas, three other members of his QUICKFIX platoon died in the crash, including platoon leader 1st Lt. Donnie Tillar (a UH-60 pilot), CWO Jack Morgan (a UH-60 pilot), and S. Sgt. Jonathan Kamm (a UH-60 repairman).
