NOTE 2:
On 31/03/1991, RF-4C 64-1056 with a crew of Pilot Cpt. John Norman and WSO Cpt. Jeff Kregel, both from 106th TRS/117th TRW, experienced bleed air duct failure and a catastrophic engine failure right after take-off from Shaik Isa AB, Bahrain. They ejected and were rescued.
BUT, until now, I was not able to find how. After reading Rotor Review magazines from the past, now I am able connect the dots. SH-3G BuNo 148047/HU-744 "Wild Duck" from HC-2 was on a routine mail/cargo run from Manama, Bahrain, when they heard "Mayday" on a Guard frequency. Crew consisting of Helicopter Aircraft Commander LT Jack "Sluggo" Makdad, Copilot LTJG Tim Mattison, swimmer AMS3 Glen Mustian and first air crewman ADCS Steven Fears responded immediately and were able to rescue the crew which showed signs of hypothermia. Ongoing coordination showed that the closest medical facility is Bahrain Defence Hospital in Riffa, Bahrain. A quick stop at Shaik Isa AB was made where two medical personnel boarded the helo. After takeoff, "Angel 744" was joined by BDF helo which escorted them to the hospital.
